§ 2-151. Form for damage claims; service of claim.

All claims for damages against the city, or any board, commission, employee or officer shall be in writing and presented to the city clerk or other person specified in A.R.S. § 12-821.01. The writing shall set forth with sufficient particularity the event, occurrence or transaction on which it is based so that it may be properly investigated and shall also set forth the name and address of the claimant and authorized representative, if any, the time, date, place and circumstances of the event, occurrence or transaction, the extent of the injuries or damages sustained, and the amount of any monetary damages claimed.
