§ 16-22. Application for license.

  • (a)

    Upon a person making any required application for the first license to be issued under this article for a newly established business, such person shall furnish to the City Manager a signed statement, upon a form provided by the City Manager, setting forth the following information:

    (1)

    The name and exact nature or kind of business for which a license is requested, whether a merchant business or a service business;

    (2)

    The place where such business is to be carried on and, if the same is not to be carried on at any permanent place of business, the places of residence of the owners of same;

    (3)

    The business owner's name, address, and contact information. In the event that application is made for the issuance of a license to a person doing business under a fictitious name, the application shall set forth the names and places of residences of those owning the business;

    (4)

    In the event that the application is made for the issuance of a license to an entity, the application shall include the names, addresses, and contact information for all of the officers or partners thereof;

    (5)

    For individuals, proof required by Arizona Revised Statutes section 41-1080(A), that the applicant is a citizen of the United States, or a non-citizen authorized to work in the United States;

    (6)

    Any further information which the City Manager may require to enable the issuance of a license applied for.

    (b)

    It is a condition precedent to issuance, renewal, and continuation of a license that the business is in compliance with all applicable state and city laws, rules and regulations.

    (c)

    A change in business address requires a new application.

    (d)

    A license must be renewed, and the renewal fee paid, on or before the last business day in January.

    (e)

    After purchase of a business, and before the business begins to operate, the buyer shall obtain a new business registration license, and provide to the City Manager the bill of sale showing the purchase of the business. If the purchase of the business includes purchase of the legal entity owning the business, and does not change the name or address of the business, within ten (10) days of the purchase, the buyer shall provide the City Manager:

    (1)

    Notice of the purchase and the bill of sale showing the purchase of the business, and

    (2)

    A new business registration license application.
